Certainly this position will be more/less comfortable for different bottoms with different levels of mobility, but no, it’s not a requirement that your bottom be able to hyperextend their elbows. Make sure that they can drop their head far enough that the arms can be moved behind the head. Then move slowly in the beginning when applying pressure to the arms to make sure that the range of motion is safe and comfortable enough.

Good question! Rope placed directly on the groin can definitely be a source of discomfort. Many bottoms will prefer the rope placed just below the groin where the tendon isn’t so exposed. You just need to be mindful to not try to place the rope too low, as it will slip up in suspension. Gorgone talks about placement a bit in their hip loader video if you’d like a visual demonstration.
